RESOLUTION OF THE TOWN OF LOS GATOS
AUTHORIZING THE TOWN MANAGER TO SIGN
AGREEMENTS WITH UNUM
FOR SHORT TERM AND LONG TERM DISABILITY AND LIFE INSURANCE
RESOLVED, by the Town Council of the Town of Los Gatos, County of Santa
Clara, State of California, that the Town of Los Gatos enter into agreements with UNUM
Life Insurance Company of America for Short Term and Long Term Disability and Life
Insurance on the terms and conditions outlined in the Short Term Disability, Long Term
Disability and Life Insurance Policies with benefits and premiums as described in attached
Exhibit A on general terms and conditions described in sample policies attached as Exhibit
B and that the Town Manager is authorized, and is hereby directed, to execute said
agreements in the name and on behalf of the Town of Los Gatos.
PASSED AND ADOPTED at a regular meeting of the Town Council of the
Town of Los Gatos, California, held on the 20th day of July, 1992 by the following vote:
