RESOLUTION OF THE TOWN OF LOS GATOS
AUTHORIZING THE TOWN MANAGER TO SIGN AN
AGREEMENT WITH PERSONAL PERFORMANCE
CONSULTANTS OF CALIFORNIA, INC.
RESOLVED, by the Town Council of the Town of Los Gatos, County of Santa
Clara, State of California, that the Town of Los Gatos enter into and the Town Manager
is authorized, and is hereby directed, to extend its agreement with Personal Performance
Consultants of California, Inc. on the terms and conditions set forth in the renewal letter
attached as Exhibit A.
PASSED AND ADOPTED at a regular meeting of the Town Council of the
Town of Los Gatos, California, held on the 17th day of August, 1992 by the following vote:

This letter is to serve as a renewal of the current contract for the Employee Assistance
Program (EAP) between Personal Performance Consultants of California, Inc. and Town
of Los Gatos, which is effective for the period of September 1, 1990 through August 31,
1992.
The new contract term will be for two (2) years, extending from September 1, 1992
through August 31, 1994. Based on an employee population of 185, the annualized
professional fee will be $10,122.45 for year one and $10,929.80 for year two, payable in
quarterly installments of $2,530.61 for year one and $2,682.45 for year two payable on the
first day of each contract quarter.
Paragraph 16.3 NOTICES is changed to read:

PPC will provide professional, experienced counselors to make assessments of
Enrollee problems and to provide short-term counseling, involving up to eight (8) inperson
sessions per problem, as clinically appropriate.
All remaining terms of the original Agreement remain unchanged and in effect.
If this letter reflects your understanding of the new terms of our agreement, please have
each letter signed and witnessed by the appropriate person and return one to Stephanie
Kushel at 1620 26th Street, Suite 2050, Santa Monica, CA 90404.
