RESOLUTION OF THE TOWN OF LOS GATES
AUTHORIZING THE TOWN MANAGER TO EXECUTE AN
AGREEMENT WITH FORTIS BENEFITS INSURANCE COMPANY
FOR SHORT TERM DISABILITY, LONG TERM DISABILITY
AND LIFE INSURANCE
RESOLVED, by the Town Council of the Town of Los Gates, County of Santa
Clark, State of California, that the Town of Los Gates enter into an agreement with Fortis Benefits
Insurance Company for Short Term Disability, Long Term Disability and Life Insurance on the
terms and conditions outlined in the Short Term Disability, Long Term Disability and Life
Insurance Policies on general terms and conditions described in sample policies (available for
review in the Clerk's Office), and that the Town Manager is authorized, and is hereby directed,
to execute said agreement in the name and on behalf of the Town of Los Gates.
PASSED AND ADOPTED at a regular meeting of the Town Council of the Town
of Los Gates, California, held on the 19th day of June, 1995 by the following vote:
