SUBJECT: PPW JOB NO. 11 -16 — FY 2011/12 STREET REPAIR & RESURFACING

AUTHORIZE THE TOWN MANAGER TO EXECUTE A CERTIFICATE OF
ACCEPTANCE AND NOTICE OF COMPLETION ACCEPTING THE WORK
OF INTERMOUNTAIN SLURRY SEAL INC. AND AUTHORIZE
RECORDING BY THE CLERK ADMINISTRATOR
RECOMMENDATION:
Authorize the Town Manager to execute a Certificate of Acceptance and Notice of Completion
accepting the work of Intermountain Slurry Seal Inc. and authorize recording by the Clerk
Administrator.
BACKGROUND:
The Town's adopted FY 2011/12- 2015/16 Capital Improvement Program (CIP) Budget
designates funding for infrastructure repair and replacement projects and is consistent with the
Town's goals to improve public safety and to improve the quality of Town streets.
This project was designed to resurface several sections of the Town with the use of the
Rubberized Asphalt Concrete (RAC) Cape Seal process, as recommended by the Council at the
May 7, 2012 Town Council meeting. The designated streets -for -this project are located- within
the general areas of Chirco Drive, Placer Oaks Drive, Longmeadow Drive, and the upper part of
the Almond Grove area.
DISCUSSION:
Two bids were received for this project in May, however, neither bid complied with the Town's
requirements and bidding process. Both bids were rejected by the Council and subsequently re-
bid during June. During the July Council recess, the contract was awarded to Intermountain
Slurry Seal Inc. under the Town Manager's Authority, and subsequently approved by the Town
Council on August 6, 2012...- --°°��

DISCUSSION (cont'd.):
The contracted work has been satisfactorily completed by Intermountain Slurry Seal Inc. and is
being recommended for acceptance by the Parks and Public Works Department.
Ten percent of the faithful performance bond will remain in effect for a period of two years to
guarantee any needed repair and /or replacement caused by defective materials and workmanship.
The execution and recordation of the Certificate of Acceptance is now required to finalize the
Town's acceptance of the project.
CONCLUSION:
Staff recommends that the Town Council authorize the Town Manager to execute a Certificate of
Acceptance and Notice of Completion accepting the work of Intermountain Slurry Seal Inc. and
authorize recording by the Clerk Administrator.
ENVIRONMENTAL ASSESSMENT:
This is a project as defined under CEQA but is Categorically Exempt (Section 15301c). A Notice
of Exemption will not be filed.
FISCAL IMPACT:
This project was completed within the Council approved budget, as demonstrated by the fiscal
table below.
