RESOLUTION OF THE TOWN OF LOS GATOS
AUTHORIZING THE TOWN MANAGER TO NEGOTIATE AND EXECUTE
AN AGREEMENT FOR WORKERS' COMPENSATION CLAIMS ADMINISTRATION
CONSULTANT SERVICES FOR~FY 2002-2003, 2003-2004, AND 2004-2005
RESOLVED., by the Town Council of the Town of Los Gatos, County of Santa Clara, State of
California, that the Town .Manager is hereby authorized to negotiate :and execute an agreement for
workers' compensation claims administration consultant services for FY 2002-2003, 2003-2004, and
2004-2005., subject to the selection criteria attached hereto as Exhibit A.
PASSED AND ADOPTED at a regular meeting of the Town Council of the Town
of Los Gatos, California, held on the 3ra day of June, 2002 by the following vote:

Workers' Compensation Claims Administration Services :Selection Criteria:
A. Established record of consistent professional service and reputation within the
industry, inchlding customer service to employees and other clients;
B. High quality references from public entities, particularly full service municipalities;
C. Established relationships with and knowledge of medical service providers in Santa
Clara County and adjacent communities;
D. Staffing and experience levels;
E. Cost effectiveness of medical and legal .cost containment services and activities;
F. Overall cost-benefit advantages; and
G. Cost not to exceed .$40,000 in FY 2002-2003, $42.,000 in FY 2003-2004, .and
$44,000 in FY 2004-2005.
