As a new endeavor, a number of our outcome assumptions are based on the experience of other
2-1-1 call centers serving comparable populations, i.e. number of calls at launch and increasing
going forward. As 2-1-1 SCC's role in the community evolves, different criteria will shape the
expanded objectives and measurement and evaluation of outcomes, i.e. ending homeless
collaborations.
We will focus on measuring 2-1-1 SCC's impact on the targeted and intended audiences,
including but not limited to, the following: Individuals, Community Service Providers,
Employers, Existing Information and Referral Services, and Funders and Planners.
The current comprehensive evaluation plan will utilize software survey tools that will assist in
data reporting. Our projections on anticipated client service do not reflect the additional hits
through the public website that will be launched in May 2007. The current scope of services
focuses on general calls for information and referrals on existing services and programs, with the
exception to data being collected on the targeted 0-5 years of age population (specific funded
focus). A critical component of 2-1-1 SCC will be its ability to collect data about Santa Clara
County resident needs, unmet needs, and gaps in service, particularly as they relate to the health
and human service sector.

Dear Council Members, Town Staff, and the community of Los Gatos,
A Place For Teens, the organization that provides the Venue to the teens of Los Gatos and the
west valley, would like to thank our elected leaders of Los Gatos for their continued generous
support in so many ways, not the least of which are the community grants.
Our mission is to provide a safe environment where teens can build self esteem, learn to make
healthy decisions, find positive alternatives to risky behavior and develop a growing sense of
community. This year the Venue is thirteen years old and showing the wear that from the 300+
active teens we serve on average each day. Some of our most popular teen programs at the venue
have been concerts, "open mic"' nights, and others related to music. With the help of the Town
and other community groups we recently added a recording studio to the Venue's attractions,
providing a unique service to the youth in our community that speaks directly to their passion for
music..
Even as we try to expand the programs we offer, and welcome the increasing numbers of teens
that enjoy the Venue, we find ourselves dealing with the economic realities of our success.
Other than the community grant finds from the Town of Los Gatos and a small grant from Monte
Sereno, A Place for Teens receives no government funding, and relies completely on community
donations and our own fundraising activities.
III the last five years our world has changed in many ways, not the least of which are the
economic realities that have left few in our community untouched. The number of corporate and
individual givers to not-for-profit organizations has declined significantly over this time. A Place
for Teens has faced increasing competition fi-om new local organizations raising money for
educational programs, for far fewer available donation dollars in the bay area and California.
Despite this we made the decision to spend from our savings to maintain the level of service to
teens while the donations we received continued to fall. Unfortunately, we no longer have this
option.
Despite the tremendous support we receive from our community, especially local merchants, and
the success achieving our mission to serve teens, A Place For Teens is at a crossroad. In response
to this economic challenge we have elected to fundamentally change how we manage ourselves
and to set even higher goals for service and fundraising. We are actively seeking individuals for
our board of directors who have the energy, skills and the community connections we need to
take our teen programs and our fundraising to a new level.
Los Gatos and the West Valley is a community of capable, resourceful and successful individuals
who understand that we have unique community. Los Gatos is special place precisely because we
are a community that gets involved, that gives of ourselves, our time , and our personal
resources. A Place For Teens is looking for those in the community with experience who would
be willing to mentor us in our changing leadership, join us in growing our programming and
fundraising, and if possible, support our efforts financially.
