ACCEPTING WORD OF MCH GENERAL ENGINEERING CONTRACTORS, INC.
PROJECT NO. 00-12: HOLLYWOOD/YOSEMITE/WHEEI,ER AVES.
PROJECT NO.00-24: MASSOL/NICHOLSON/TAIT/HERNANDEZ AYES.
PROJECT NO.00-35: CYPRESS/HIGH STREETS
PROJECT NO.00-36: LORA DRIVE
PROJECT NO.00-39: BACHMAN PARK
AND AUTHORIZING THE TOWN MANAGER/EXECUTIVE DIRECTOR
TO EXECUTE
CERTIFICATE OF ACCEPTANCE .AND NOTICE OF COMPLETION
FOR RECORDING SY THE TOWN CLERK
WHEREAS, on August 7, .2000, the Town Council/Redevelopment Agency awarded a
construction contract to MCH General Engineering Contractors, .Inc. for the following storm
drain improvement projects:
PROJECT NO. DO-12: HOLLYWOOD/YOSEMITE/WHEELER AVES.
PROJECT NO. 00-24: MASSOL/NICHOLSON/TAIT/HERNANDEZ AYES.
PROJECT NO.00-35: CYPRESS/HIGH STREETS
PROJECT NO. 00-36: LORA DRIVE
PROJECT NO. 00-39: BACHMAN PARK
and
WHEREAS, MCH General .Engineering Contractors, Inc. has satisfactorily completed all
work under said contract.
NOW, THEREFORE, BE IT RESOLVED THAT:
1. All work completed by MCH General Engineering Contractors, Inc., on said projects
is hereby .accepted as complete pursuant to the plans .and specifications for each project.
2. The Town Manager/Executive Director is authorized and directed to execute the
Certificate of Acceptance and Notice of Completion attached as Exhibit A hereto.
3. The Town Clerk is directed to transmit this Certificate of Acceptance and Notice of
Completion to the Recorder of Santa Clara County for recording.
PASSED AND ADOPTED at a regular meeting of the Town Council/Redevelopment
Agency of the Town of Los Gatos, California, held on the 17t'' day of December,. 2001 by the
following vote:

We do hereby certify that MCH General Engineering Contractors, .Inc. substantially
completed the work on December 17, 2001, called for in the original contract with the Town of
Los Gatos dated August 7, 2000. The work is listed in the specifications prepared by the Town
of Los Gatos and generally consisted of furnishing all labor, materials, tools, equipment, and
services required for completion of Storm Drain System Improvements in the TOWN OF LOS
GATOS, County of Santa Clara, State of California and was completed, approved and accepted
on December 17, 2001..
The Surety on the bond accompanying the contract is as follows:

I, DEBRA J. FIGONE, Town Manager of the Town of Los Gatos/Executive Director of the
Redevelopment Agency of the Town of Los Gatos, have read the. foregoing CERTIFICATE OF
ACCEPTANCE AND NOTICE OF COMPLETION and know the contents thereof. The same is
true of my own knowledge, except as to the matters which are therein alleged on information or
belief, :and as to those matters I believe it to be true.
I declare under penalty of perjury that the foregoing is true and correct and that this declaration
was executed on 2001 at Los Gatos, California.
